Pharmacological effects
1. This product is a nucleoside reverse transcriptase inhibitor. As precursor, this product in the blood plasma rapidly hydrolyzed into free tenofovir, which is a kind of open loop nucleotide phosphate, structure and function are similar to adefovir, is a retrovirus, addicted to liver DNA viruses, including HIV - 1) the potent inhibition of medicine, in the peripheral blood mononuclear cell (PBMC) and melatonin (MT) - 2) 2 cells of HIV - 1 50% inhibitory concentration respectively tendency tendency for 0.2 L and 0.7 L.
 
2. Unlike nucleotide analogues such as Zidovudine, tenofovir and adefovir undergo no initial intracellular phosphorylation process and are rapidly converted into their corresponding active diphosphate forms. Bisphosphonate form is a powerful intracellular inhibitor of retrovirus reverse transcriptase, which can be maintained in HV infected cells for a long time in vitro, indicating sustained inhibition of HIV replication.
 
3. In vitro studies, the product was rapidly transformed into tenofovir in cells and produced a higher concentration of tenofovir diphosphate than tenofovir diphosphate.
 
Pharmacokinetics
1. Absorption: This product is a water-soluble diacid ester of tenofovir, the active ingredient, and is a precursor drug. Oral bioavailability was about 25%. For HIV-infected patients, the single dose of the drug was taken on an empty stomach for 300mg. After (1.0±0.4) h, the Cmax was (0.30±0.09) pg/ mL, and the AUC was (2.29±0.69) (g·h) /ml. The dose was between 75-600mg. The oral powder was 26% lower than the tablet Cmax and the AUC was similar.
 
2. Distribution: In vitro test showed that the binding rates of plasma and serum protein were lower than 0.7% and 7.2%, respectively, at the concentration of 0.01 -- 25 g/ mL. Intravenous administration of 1.0mg/kg and 3.0mg/kg was performed, and the distribution volumes were about 1.2l /kg and 1.3L/kg, respectively.
 
3. Metabolism and elimination: In vitro studies have shown that neither tenofovir dipivoxil nor tenofovir dipivoxil are CYP substrates. After intravenous administration, 70%-80% tenofovir was recovered in urine within 72h. Single dose oral administration, t1/2 for 17h. Tenofovir can be cleared by hemodialysis and the extraction coefficient of hemodialysis is 54%. 300mg once per day, 24h after multi-dose oral administration (after eating), 32% 10% of the dose was recovered in urine. Tenofovir is actively excreted through glomerular filtration and renal tubules and may be competitively excreted with other drugs excreted through the kidneys.
 
4. Influence of food: 300mg of the tablet was given after a high-fat meal to increase the bioavailability of the tablet, and the AUC and Cmax of tenofovir increased by 40% and about 14% respectively. Low fat diet had little effect on pharmacokinetic parameters. Eating can delay TMAX by 1h. Take 300mg after eating. After one dose per day, the Cmax and AUC were (0.33±0.12) g/ mL and (3.32±1.37) (g·h) /ml, respectively.
 
5. Renal function has a great influence on the pharmacokinetics of this product: the pharmacokinetics parameters after a single dose of 300mg are shown in the table below.
